Our protocol for TXA loading dose is 1g in 100 ml over 10 mins however in reality most slow IVP it over a few minutes. The over 10 mins is due to concern with transient hypotension possibly associated with rapid pushes in the CRASH trial however these patients were shocky and hypotension was likely multi-factorial.

In most major trauma the tendency has been to use 1:1:1 ratio of plasma (as FFP), platelets and red cells, however as I understand, this has evolved somewhat to just giving whole blood. Who is going to prime and run pump, will they bring a perfusionist as the doc is assumably cannulating? Since it is a field response you have to bring everything with you. PRBCs, plts, FFP, cryo, bicarb, lytes, heparin, vit k, amicar, txa, and so on. How do you run coags, let alone a TEG in the back of a bus? What about a CBC?

TXA and aminocaproic acid are useful when there is tissue destruction which releases clot dissolving enzymes. Unless there was super imposed trauma in this case, it wouldn't add anything to the treatment. There is no telling where else a patient like this would bleed. Maybe he just bit his tongue. Never seen an INR that high from Coumadin.
