In my town we've gotten 4 or 5 tornado warnings in the last few years and not one was an actual seen tornado it was "radar indicated".

If you read the text, they either say "source = radar indicated rotation" or "confirmed tornado ". Fox weather live TV shows the warning polygons with confirmed tornado in purple, radar indicated rotation warnings in red.
