The Clinical Global Impressions scale is very commonly used in clinical trials in mental health. It was introduced in 1976 in a federal publication and is in the public domain. Despite, or perhaps because of, its simplicity, it is one of the most widely used scales in clinical trials in psychopharmacology. More @Wikipedia
