Not to go off on a tangent, but the "attached is/are ..." phrasing is a bit hackneyed. Why not use the active voice and say "I have attached my passport, birth certificate and diploma"? That avoids the singular/plural verb question, too.

But we attach something (physically) to something else by or with (the means of attaching it), eg, "The shelves were attached to the wall by screws." "He attached the poster to the wall with glue." (P.S., In emails, these documents are called, unsurprisingly, "attachments".)
- attach 'to' or attach 'on' - WordReference ForumsIf the tags somehow go through the clothes I would use "attach them to". If they sit on the surface of the clothes without penetrating them (which seems very ineffective) I would say "place them on".

Now, what if I'm to send this form to another person who doesn't know I'll be sending this to him but who generally approves all such completed forms, should I go with 'a':

I think the equivalent of that is to say it in the active voice: 'I have attached XXX (to this email)', or in your case 'I have attached a catalogue including our latest productions, as requested by you last week'. ('For your request' sounds strange.)
