As for the placement of the country indicator, it's generally before the symbol/amount string: "US $10", although I have seen "$10 US" in magazines as well, so (as long as you're consistent) I wouldn't worry too much. each of us. and. each one of us. Are basically identical. The former is merely removing a redundant word, but it's conveying the same thing. each and every one of us. puts more of an emphasis on making sure the document is received by everyone.

I think you are right: (3) is incorrect. The best option is (2), since U.S. is the normal way to write it, and periods are not typically doubled in English.
