A Putnam County man is facing vehicular homicide charges in a three-car crash that killed a woman in the Mahopac portion of the Town of Carmel over the weekend, New York State Police said. Police said the driver of the Jeep, a 53-year-old Carmel woman, was pulling out of a parking lot and was turning left onto Route 6 south when she failed to yield to O’Keefe’s car, which was traveling north.
