Federal investigators say the crew of the small charter jet that crashed Nov. 10 in Akron, killing all on board, did not follow proper procedures as the craft approached Akron Fulton International Airport. In addition, the “disregard for safety” extended to “organizational factors, scheduling and other practices” that predated the flight, the National Transportation Safety Board chairman said in opening statements this morning.Federal regulators also failed in oversight of the charter jet company, Florida-based ExecuFlight, the NTSB said.“The accident you will hear about today , we found flight crew, FAA, and inspectors fell short,” NTSB Board Chairman Christopher Hart said.
